U.S, Nigeria Sign $5.1bn MoU, Largest Co‑investment Under America First Strategy

The United States has signed a five-year, $5.1 billion bilateral health cooperation agreement with Nigeria, a move that signals continued strategic engagement despite recent strains in relations between Washington and Abuja.

Announced by U.S. Missions in Nigeria, the Memorandum of Understanding (MOU) with the United states commits $5.1 billion in U.S. support and new domestic health spending by Nigeria, making it the largest co-investment by any country under the America First Global Health Strategy.

The agreement is aimed at strengthening Nigeria’s healthcare system, reducing preventable deaths, and deepening bilateral ties.

The timing of the deal is notable as it comes amid widespread concern across Africa over U.S. aid pullbacks, as Washington reassesses foreign assistance spending, and follows diplomatic friction triggered by Nigeria’s recent designation as a “Country of Particular Concern” over religious freedom issues.

Against that backdrop, the scale and structure of the health partnership suggests Nigeria remains a priority partner for the United States.

Health cooperation amid diplomatic strain

Under the MOU, roughly $200 million is earmarked for more than 900 Christian faith-based healthcare facilities across Nigeria.

These clinics, while accounting for about 10% of healthcare providers, serve over 30% of Nigeria’s 230 million people, particularly in rural and underserved communities. The funding will support integrated services for HIV, tuberculosis, malaria, and maternal and child health.

Nigeria faces some of the world’s most severe public health challenges, including one of the highest maternal and child mortality rates globally and an estimated 30% of the global malaria burden.

U.S. officials say the agreement is designed not only to save Nigerian lives but also to protect American health security by limiting the spread of infectious diseases.

The deal also stands out for its emphasis on domestic co-investment, aligning with Washington’s push for partner countries to take greater ownership of development financing. Nigeria’s $3 billion commitment over five years reflects that shift and could serve as a model for future health partnerships.

While political disagreements persist, the MOU underscores a broader reality: health security remains a rare area of continuity in U.S.–Africa relations, even as aid frameworks and diplomatic priorities evolve.
